4-H Project Book Order Form – You can use this order form to ensure that each youth’s project book is ordered correctly through our office. One form per club.

Club Charter Checklist – This form is used to ensure that your club is following the Ohio 4-H guidelines for community-based clubs in the county program. If you are unable to fulfill these requirements, you will need to meet with the 4-H Extension Educator to determine how best to move forward with your club.

Yardstick/Honor Club Application
Yearly Financial Summary – This form is used to complete our 990 forms for the IRS. You must turn in this form along with a bank statement from January of last year and December of last year.
