Across County Lines Policy & Request Form – This form must be completed and turned in 30 days prior to the Marion County 4-H enrollment deadline (February 15) if you live outside of Marion County but want to participate in Marion County 4-H.
Returning Volunteer Training
To be approved as a 4-H Volunteer for the 2026 season, attendance at one of the following training sessions is required:
- December 11th at 6:00 p.m.
- December 14th at 2:00 p.m.
These trainings will be held in the Prospect Room of the County Building.

Club Charter Checklist – This form is used to ensure that your club is following the Ohio 4-H guidelines for community-based clubs in the county program. If you are unable to fulfill these requirements, you will need to meet with the 4-H Extension Educator to determine how best to move forward with your club.

Yardstick/Honor Club Application
Yearly Financial Summary – This form is used to complete our 990 forms for the IRS. You must turn in this form along with a bank statement from January of last year and December of last year.
