Across County Lines Policy & Request Form – This form must be completed and turned in 30 days prior to the Marion County 4-H enrollment deadline (February 15) if you live outside of Marion County but want to participate in Marion County 4-H.

4-H Project Book Order Form – You can use this order form to ensure that each youth’s project book is ordered correctly through our office.

Club Charter Checklist – This form is used to ensure that your club is following the Ohio 4-H guidelines for community-based clubs in the county program. If you are unable to fulfill these requirements, you will need to meet with the 4-H Extension Educator to determine how best to move forward with your club.

Winning 4-H Plan – This form is used to request accommodations for youth with disabilities.
Yearly Financial Summary – This form is used to complete our 990 forms for the IRS. You must turn in this form along with a bank statement from January of last year and December of last year.
